Fax the referral to 907-331-0490, email it to admin@kphhh.org or call 907-260-4444. Here’s what helps us start care quickly.
Home health referral
- Order for home health and the services needed
- Face sheet with insurance information
- Recent history and physical, and discharge summary if any
- Medication list
- Notes from the face-to-face visit that support homebound status and skilled need
Hospice referral
- Order to evaluate and admit to hospice
- Face sheet with insurance information
- Terminal diagnosis and recent notes that support a prognosis of six months or less
- Medication list and code status, POLST or advance directive if available
- Name of the attending provider who will stay involved
Patients and families can also call us directly. We will contact your provider for the order.
Our service area
We serve patients in Soldotna, Kenai, Sterling, Nikiski and Kasilof. Clam Gulch and Cooper Landing are served when we have the staff and support to do it well.
